Late reporting of an injury Out of work for more than 30 days for a minor injury Unwitnessed injury Injury occurs in a location where the worker would not normally be Employed for less than 30 days Hard to reach at home while out of work due to injury Cancels or does not show for doctor appointments History of multiple prior claims Lack of cooperation with process Early attorney involvement
